Subject: Request for Access to Personal Medical Records
Dear [ Ulverston Community Health Centre ]
& [ Dr G R Murray & Partners ] Team,

I am requesting access to my personal medical records, specifically my recent test results, in accordance with my rights under the Data Protection Act 2018 and UK GDPR. I understand the need for identity verification; however, I do not currently have photo ID.

NHS guidelines indicate that reasonable alternative methods of identification should be considered where photo ID is unavailable. I am happy to provide alternative verification, such as confirming personal details (e.g., date of birth, NHS number) or answering security questions, to satisfy any necessary checks.

According to NHS Digital and the ICO, data controllers must take a proportionate approach to verification, ensuring that access to personal data is not unreasonably withheld. I trust that reasonable accommodations will be made in line with these principles.

If there is a specific policy requiring photo ID exclusively, please provide a copy for my reference. Otherwise, I kindly ask that reasonable alternative verification be accepted so that I may access my records as per my legal rights.
I appreciate your time and look forward to your response.

Kind regards,
[Patience Name]

Took my partner for a scan. It is very welcoming and well organised. Didn't have to wait long and no problems encountered except for the confusing car park signage.

Usually very good. That's why I haven't changed services atfer moving home. Today for at least three weeks in a row there's no lift again. The practice manager needs to get on this. Disabled people are struggling enough. Lift breakdowns are regular here throughout the year. So I have been left downstairs to await my nurse. The downstairs receptionist didn't want to ring upstairs instead sending my ambulance driver up the stairs to book me in. This place is ALL NHS so they should work together.
